Patient eligibility requirements to participate in GUIDE
Eligible patients
Traditional Medicare beneficiaries with a confirmed dementia diagnosis (including Alzheimer's, LBD, FTD, vascular dementia, etc.)
Living at home or in the community
Not Eligible
Patients with mild cognitive impairment
Medicare Advantage enrollees
Living in a skilled nursing facility
Participating in Hospice or a PACE organization
What is GUIDE?
Guiding an Improved Dementia Experience (GUIDE) is a new CMS program that focuses on improving the quality of life of patients living with dementia and their caregivers at no additional cost. Beginning July 1st, this 8-year initiative focuses on dementia care management to reduce strain on unpaid caregivers and enable people living with dementia to safely remain in their homes and communities. This program presents a unique opportunity to enhance patient health outcomes, alleviate financial strain, and bridge gaps in access to care, particularly in underserved areas, ensuring equitable support for all individuals affected by dementia.
How does GUIDE benefit patients?
GUIDE provides patients with no cost coverage for additional services many of which are incremental to what they receive in primary care today.
There are nine services that Neuroglee is required to provide patients in GUIDE:
- Comprehensive assessments including home safety evaluations
- Caregiver education and support
- Care planning for both patients and caregivers
- Medication reconciliation and management – done in close coordination with their primary care provider
- 24/7 access to their Neuroglee care team
- Care coordination and transitional care management
- Referrals and coordination to community support services and supports
- Up to $2,500/year in Respite Services paid for by the CMS GUIDE program
- On-going monitoring and support
Nearly all GUIDE model services are part of Neuroglee's care model making it easy for Neuroglee to support patients in GUIDE.
Does Neuroglee compete with primary care for patients, attribution or fee for service billing?
Not at all. Attribution stays with the primary care provider. Neuroglee offers additional support serving as primary care's dementia care care 'extension' that improves patient experience, outcomes and costs. Having patients enrolled in GUIDE with Neuroglee does not affect your ability to bill for services including CCM.
Does GUIDE improve value-based care performance?
Neuroglee partners with primary care organizations to leverage GUIDE to improve performance in MSSP and ACO Reach by reducing the total cost of care for patients with dementia.
